Not every employee who may be counted for the purposes of meeting the required ratios has a valid certification in first aid, including infant and child CPR, issued by a WSIB-approved training course.

The licensee has not kept a daily attendance record showing the time of arrival and departure or the absence of each child.

Objections to the immunization of a child or a medical practitioner's medical reasons as to why the child should not be immunized have not been submitted in a form approved by the Minister.
